Qualifications

· Required bachelor’s degree required upon hire

· CRT-Basic Life Support (BLS) - AHA American Heart Association Pennock 

Responsibilities:

· Assesses, monitors, detects, plans and takes appropriate action to prevent potential physiological complications associated with wound, ostomy or continence

· Provides information and instruction to patients and their caregivers

· Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community

· Participates in projects related to area of Wound, Ostomy and Continence Nursing and developing and evaluating educational materials and programs and policies

· Assesses and monitors the overall physical needs of the patients/family in regards to ostomy, wound and continence; Evaluates each individual patient’s response to treatment

· Assists in developing/maintaining an individual plan of care with the patient, consistent with changing needs and goals reflecting full scope of practice

· Collaborates in discharge planning for patient at time of patient contact, including families, caregivers as appropriate

· May facilitate a smooth transition to the next point of service/home including referrals 

· Assesses the educational needs of a select group of patients, caregivers, and staff

· Ensures that required documentation for activities within area of responsibility is performed in an accurate and timely manner

· Collaborates with nurses, providers, and other departments to develop, implement and evaluate programs and services that support patient care delivery in Wound, Ostomy and Continence Nursing

· Collaborates with nursing quality department to develop and maintain quality monitoring system to continuously monitor and measure the quality, safety and outcomes of nursing services for assigned area
